• Legal entity name (matches MC filing)
• MC number + DOT number
• EIN
• Physical + mailing address
• Years in business (state "new authority" if under 1 year)

Operating info

• Radius of operation (local / regional / OTR)
• Commodities you plan to haul
• Estimated annual miles per truck
• Do you plan to haul: HHG? Hazmat? Reefer? Auto transport?

Truck info (per truck)

• VIN
• Year, make, model, GVWR
• Purchase price / stated value (for physical damage)
• Owned vs leased vs financed (lienholder if financed)

Driver info (per driver)

• Full legal name
• Date of birth
• License #, state, class (CDL / non-CDL)
• Years of driving experience
• MVR — get a copy, they'll pull one too
• Any accidents / violations in last 5 years

Coverage minimums to quote

• Auto liability: $1,000,000 (FMCSA min is $750k but $1M is standard)
• Cargo: $100,000
• Physical damage: truck ACV
• General liability: $1,000,000
• Non-trucking liability (bobtail)
• Trailer interchange (if pulling shipper's trailer)
